5 scary times for your heart

Our resident cardiologist, Arthur Agatston, MD, told Prevention that the first question he asks patients when they walk in his office is "How's the traffic?"

It's not small talk but a valid medical query. Being stuck in traffic raises blood pressure and triples heart attack risk. So if a patient has had a tough commute and her BP is elevated, he'll recheck it later in the appointment. There are other surprising situations and times when the chance of heart attack rises dramatically. If you or someone you know has a history of heart trouble, here's when to be watchful.

    1. First thing in the morning

The risk of heart attack increases 40% in the morning, Harvard researchers estimate.

As you awaken, your body secretes adrenaline and other stress hormones, increasing blood pressure and a demand for oxygen. Your blood is also thicker and harder to pump because you're partially dehydrated. All this taxes the heart.

Protect yourself: Build some time into your schedule so you can hit the snooze button and wake up slowly. If you're a morning exerciser, warm up thoroughly so as not to additionally stress the heart. And if you're on a beta-blocker, take it before bed so the medication is at full strength in the am.

    2. On Monday mornings especially

Science shows there's good reason to dread the first day of the work week.

Twenty percent more heart attacks occur on this day, probably because people are stressed and depressed about returning to work.

Protect yourself: Relax on Sunday, but try not to sleep in. Getting up early on Monday after sleeping late Saturday and Sunday can raise blood pressure even more because your body is fatigued and its natural rhythms are out of whack. Try to maintain a regular sleep/wake schedule all week.

    3. After an indulgent meal

A five-course, calories-be-damned dinner can have an immediate impact on your heart health. Studies show that high-fat, high-carb meals constrict blood vessels, making blood more prone to clotting.

Protect yourself: If you must indulge, keep your portion sizes reasonable. A daily aspirin will also help prevent blood "stickiness."

    4. During unusually vigorous exercise

Having a heart attack while shoveling snow is a classic example of this.

The heart attack occurs because the victim isn't accustomed to that kind of effort and stress hormones skyrocket, causing blood pressure and heart rate to jump.

Protect yourself: Regular exercise protects your heart. But increase your intensity level gradually.

    5. At the podium

From the heart's perspective, public speaking can be similar to unaccustomed exercise.

Extreme nervousness raises blood pressure, heart rate, and adrenaline levels, all of which can make the presentation itself a secondary worry.

Protect yourself: To counter these effects, some of my patients take a beta-blocker before speaking, flying, or doing anything that makes them overly anxious.

Shoveling snow is harder than hell! A couple inches--not a big deal. But 12+ inches? Very difficult--plus you're out in the cold air and usually overdressed... AND people usually try to it all done fast in one session. Spread it out. Stretch before you do it. Take your time. Take breaks.

I'm quite fit walk/Spin/pilates--but shoveling even my short walkway w/12 inches of snow was a big challenge. I really noted my heartrate increasing... Plus, you're in an awkward position while shoveling.

Prime example of what this Country doesn't need. Someone telling us that when we wake up we're 40% more likely to have a heart-attack. This Country worries about everything, down to things that we shouldn't even stick out noses in. You want to prolong life, well you can't. Death is inevitable and the quicker people realize this, the better off your life will be. Life happens people! I work at a hospital and I see people who run everyday, eat all food groups daily, don't smoke, don't drink and, BOOM, heart-attack. As a human race we need to stop trying to control every little thing in our lives and just enjoy being on this planet. If you want a 5-course meal, go for it! You want to sleep in until Noon Saturday and Sunday, DO IT. Stress is the number one cause of heart-attacks, which means high blood pressure for the most part. When you read articles like this, don't you worry if "today's gonna be the day"? That's stress people. We need to stop worrying about who said RETARD or S*IT on TV and start remembering that we can't control everything that happens, so stop apologizing for this little crap and move on with your lives. I deal with you people everyday, and if you guys just had a little bit of fun in your lives and appreciate what you have, you'll be fine.

Well I learnt a lot from that. You are more likely to have a heart attack when moving around quickly after a long period of not moving at all, so your heart is working hard. When exercising vigorously, so your heart is working hard. When your heart has to move blood to your digestive system away from your organs, and so your heart is working hard. When your anxious, so your heart is working hard.

Very worthwhile article. I hope you were paid a lot of money for it.

Thank you, Jennifer! Joy, you must not know how heavy snow is especially when the temperature fluctuates and it becomes heavier not to mention the snow plows that leave a thick 2 ft. pile at the end of everyone's driveway. It's funny that someone can praise their own physical fitness when they haven't experienced it, there is a reason it was mentioned in the article. The temperature is often cold meanwhile your body is warm and stressed. Cleveland was named the worst winter city this year. I don't believe Seattle was anywhere near the top. Anyways, I'm glad (Cleveland) winter is almost over.

Last monday i had a big breakfast first thing in the morning before i went out to shovel my driveway for the big speach at work
    • Jennifer  •  2 years 3 months ago
Obviously Joy - since you live in Seattle & probably get no snow just rain you don't know anything about shoveling snow which can tire out a fit person in a matter of minutes if the snow is wet & heavy. My father died of a sudden massive heart attack moving heavy bales of hay. He was physically active every day of his life - but he was over-exerting himself. The same thing can happen & does when people shovel snow.

Wow, so just waking up is a heart risk? According to American doctors, there is NO right way to life, EVER!

P.S.S. The Aspirin a day is true, oh so true. I am a RN and that is the one important question we ask our pt when he/she comes in while having a heart attack; did you take an aspirin when you first began with the pain?? I cannot tell you the odds of survival if the pt tells us that yes indeed they took a simple 81mg aspirin (a baby aspirin), it saves their life. Yes, saves their life!!!!!!!! Take your aspirin, 81mg daily, on a full stomach, it could save your life.....it is not stupid or crazy, ask any educated health care professional! No matter how stupid some will think I am, I WILL continue to take my 81mg ASA every day. I hope you do too.

I have inherited my hypertension from my father and mother. I lost my oldest brother lastyear (he was just 51) and he never went to the doctors for his sleep apnea and at his employment physical they saw he had high blood pressure. he was a truckdriver (very stressful job)
Our lives are never the same after losing someone to something that is treatable. We just have to live better and take care of ourselves. god bless us all :)

Joy, I'm willing to bet you are very young, not only because of your offensive language, but because of your assumptions. Doctors recommend daily exercise for older people, and most are pretty good about getting it (older people are actually better about it than young people, who assume they are immortal, are). Shoveling snow, however, is usually something they warn older people against. Part of that is that since it doesn't snow every day, people usually don't have an opportunity to work up to it. No one recommends that anyone get off the couch and run a marathon the first time out. The last time it snowed, it took me about 3 hours to get my driveway clear. I can do it because I'm in my 50s, but generally those people I know who are older hire someone to do it for them, including those who spend a lot of their time involved in sports and who exercise regularly.

Taking an aspirin everyday violates (contradicts) the manufacturer's
dose prescription (what's written on all the labels) and it also is
not valid advice: too thin of blood can induce a heart attack just
as fast as too thick. Before a person decides to self-prescribe
any medication (over the counter or otherwise), it would be
most prudent to first have a blood chemistry done by a
qualified (licensed) medical doctor; a blood chemistry
can altercate naturally from day to day (depending
upon exercise and consumption behavior) but at
least one would have some concept of whether or
not a medication like aspirin (buffered arsenic:
arsenic toxidity can be fatal and taking aspirin
daily can cause such toxidity or accumulative
deterimental effect) could be tolerated by
      the individual...
